Last summer I planted Aesclepias incarnata in large numbers on our acre property. This summer I see monarchs daily--all summer, in all stages: adults, eggs, caterpillars, chrysalises, and "shed" chrysalises. We have a monarch nursery here. The chrysalises are hanging from plants; under the patio railings; even on the corners of our window shutters!

I wouldn't know where to begin filling out your questionnaire--we have all stages and in pretty large numbers drawn to our native plant gardens.
